Welcome to Crossroads Animal Care Center
Thank you for choosing Crossroads Animal Care Center! Below you will find forms to help facilitate your visits here, giving you more time with the veterinarian or making dropping off your pet quickly and easy.
Every new patient to Crossroads will need a completed Welcome Form followed by the Animal Medical Care Disclosure Form to be kept with his or her medical records. We encourage you to bring these completed forms as well as any and all prior medical history or paperwork on your pet.
While we understand not all information can be delivered prior to the scheduled appointment, if your pet has been to a previous vet we would love to be able to contact them to obtain prior records. This helps us prepare your pet’s record and get you as much time with the veterinarian as possible.

Preparing for Your Visit
In order to make your appointment process quicker and easier for you and your pet, please fill out our new client forms online before your visit!
To help us learn as much as possible about your pet and his or her medical history, please obtain any previous health records that you might have. Or, if you prefer, just notify us and we can obtain the records for you in advance of your visit.
